Digital School Forms

 

Madison Central School District will be utilizing a digital method for beginning of the year documents.  This will greatly reduce the number of physical copies of paper that you will need to sign.

Below you will find links to the most common documents that need to be filled out by parents/guardians at the beginning of the school year.  You will need to create an account in Arbiter to complete and sign these digital documents.  Please follow these links to complete the begining of the year enrollments for your child.
